Attention 6th Grade Parents & Guardians!
As we prepare for the start of the 2026–2027 school year, please make sure your incoming 6th grade student has all required health documentation on file.
Physical Exam: Due August 18, 2026
Required Immunizations (Tdap & Meningococcal): Due August 18, 2026
Dental Exam or Waiver: Due May 15, 2027
Please note: Students who are newly enrolled in Mascoutah School District 19 will have 30 days from their first day of attendance to provide the required health documentation. Students who were previously enrolled in Mascoutah School District 19 must have their physical and required immunization documentation submitted by August 18, 2026.
Completed records may be emailed to our school nurse at hodgee@msd19.org or dropped off at the Mascoutah Middle School main office.
